Hospice Care Basics

What is Hospice?

Hospice care is a specialized form of healthcare that focuses on providing comfort, support, and quality of life for individuals facing a terminal illness. The primary goal of hospice care is to enhance the patient’s overall comfort and help them maintain dignity and peace during the final stages of their illness.

Key aspects of hospice care include:

  1. Comfort and Symptom Management: Hospice care aims to manage pain and other symptoms associated with the terminal illness. This involves a holistic team approach that addresses physical, emotional, and spiritual needs.
  2. Quality of Life: The focus is on improving the quality of life for both the patient and their family by offering support and resources. This may include counseling, spiritual care, and assistance with daily activities
  3. Patient-Centered Care: Hospice care is personalized to meet the unique needs and wishes of the individual and their family. The care plan is developed by the care team in a collaborative manner to address all patient and family needs.
  4. Emotional and Spiritual Support: Hospice care includes counseling and support services for patients and their families, helping them navigate the emotional and spiritual aspects of the end-of-life journey.
  5. Bereavement Support: After the patient’s passing, hospice provides bereavement support for the family and loved ones for 13 months, helping them cope with grief and loss.

Hospice care is the right choice when curative treatments are no longer effective or when a patient decides to focus on comfort and quality of life rather than aggressive medical interventions. It involves a multidisciplinary team of healthcare professionals, including physicians, nurses, social workers, chaplains, and volunteers, working together to provide comprehensive care and support during this challenging time.

Primary Levels of Hospice Care

Hospice care covers different levels of care to meet the varying needs of patients and their families. The primary levels of hospice care include:

  1. Routine Home Care:
    • Description: This is the most common level of hospice care and is provided wherever the patient calls home.
  2. Continuous Home Care:
    • Description: This level of care is provided during brief times of crisis when the patient is experiencing uncontrolled symptoms that require more intensive and continuous attention.
  3. Inpatient Respite Care:
    • Description: Respite care is designed to provide a short-term break for the patient’s primary caregiver(s) by admitting the patient to an inpatient facility for a brief period.
  4. General Inpatient Care (GIP):
    • Description: GIP is provided when a patient’s symptoms cannot be managed in a home setting, requiring a higher level of medical attention and is provided in a hospital or skilled facility. This is a short term and intensive level of care.

The choice of the appropriate level of care depends on the patient’s condition, the severity of symptoms, and the level of care needed to achieve the optimal level of comfort for the patient. The hospice team collaborates with the physician to determine the most suitable level of care.

Care Team

The hospice care team is a multidisciplinary group of professionals working together to provide comprehensive and compassionate care to individuals and their families who are facing a terminal illness. The composition of the hospice care team typically includes the following key members:

  1. Hospice Physician:
    • Role: The hospice physician oversees all aspects of care.
  2. Registered Nurses (RNs) and Licensed Practical Nurses (LPNs):
    • Role: Nurses manage all aspects of the patients care plan and report to the Hospice physician. They also provide guidance and education to both the patient and family on symptom management and caregiving.
  3. Certified Nursing Assistants (CNAs) or Home Health Aides:
    • Role: CNAs or home health aides assist with daily activities such as bathing, dressing, and personal care. They provide essential support to both the patient and family, contributing to the overall comfort of the patient.
  4. Social Workers:
    • Role: Social workers address the emotional and psychosocial aspects of hospice care. They offer counseling, help with advance care planning, and provide support for the patient and family in coping with the challenges of the end-of-life journey.
  5. Chaplains or Spiritual Counselors:
    • Role: Chaplains or spiritual counselors provide spiritual support based on the patient’s beliefs and values. They can assist with religious practices, offer emotional support, and help individuals find meaning and peace during this time.
  6. Volunteers:
    • Role: Trained volunteers offer companionship, respite care, and general support to patients and their families.
  7. Bereavement Counselors:
    • Role: Bereavement counselors provide support to the family and loved ones after the patient’s passing. They help individuals navigate the grieving process and offer counseling and resources to cope with loss.
  8. Therapists (Occupational, Physical, or Speech):
    • Role: Depending on the patient’s needs, therapists may be involved to address issues related to mobility, communication, or other specific therapeutic requirements.
  9. Hospice Administrator or Manager:
    • Role: The hospice administrator or manager oversees the overall operations of the hospice program, ensuring that services are coordinated effectively and meet regulatory standards.

The care team works collaboratively, meeting regularly to discuss and adjust the care plan based on the patient’s evolving needs. This multidisciplinary approach ensures that the patient receives holistic and individualized care, addressing all physical, emotional, spiritual, and social aspects of their well-being during the end-of-life journey.

Whether helping to accommodate special requests or answering a call from a caregiver in the middle of the night, our hospice nurses are there to support you and your family.

As the loved one of a person with a life-limiting illness, you want to make sure that you are there – to comfort them, to take care of them, and to make each moment meaningful. Caring for a loved one with a life-limiting illness can be challenging, however, both physically and emotionally. Sometimes a brief “respite” to recharge can make all the difference.

In-Patient Care

ViaQuest partners with high-quality, local nursing facilities that provide 24-hour skilled nursing services. All facilities offering respite care must meet the nursing needs of our patients and ensure that care is furnished in accordance with each patient’s hospice plan of care. Respite Stays can last anywhere from 1 to 5 days per occurrence and are covered by your loved one’s hospice benefit.

The Importance of Advanced Care Planning

Preparing for the end of life is uncomfortable for most people. But by planning ahead, you can make things less stressful for you and for your loved ones when that time comes. By simply completing advanced directives, you can document your end-of-life decisions in a legally sound way.

Make Life Easier on Your Loved Ones

Advanced Directives are a way to ensure your voice will be heard if you can no longer speak. With Advanced Directives, you can express how much care you do or don’t want at the end of life. You can also name someone to make decisions about your medical treatment if you are unable to make those end-of-life decisions yourself. 

Advanced Directive Documents

All states have laws regarding Advanced Directives, so choosing the right state is important. Documents must comply with the laws in your state. Once you make your decisions, it’s important that you sign and notarize these documents and give copies to your family, caregivers and health care providers.

You can change or cancel advanced directives at any time provided you are considered competent and able to clearly indicate your desires, and state laws are followed. These changes or cancellations should also be signed and notarized, and copies given to your family and health care providers to ensure they’re aware of the changes.
