Hospice services are available to patients living in ViaQuest Hospice’s service area, wherever they call home—including private residences, assisted living settings, or skilled nursing homes. To be eligible, a physician must determine that the patient has a life expectancy of six months or less, assuming their disease follows its expected course. This prognosis is a key factor in determining eligibility for hospice benefits, allowing for a focus on comfort and quality of life during this sensitive period.

Admission to ViaQuest Hospice is based on a few key criteria designed to ensure that our services align with the patient’s needs and goals. Patients must be seeking palliative (comfort) care rather than curative treatment, signifying a shift in focus towards symptom management and enhancing well-being. Additionally, patients should have a designated attending physician who is willing to collaborate with the hospice team. If a patient does not currently have an attending physician, ViaQuest Hospice can help facilitate the choice of a hospice physician to fulfill this role, ensuring seamless coordination of care. A responsible caregiver must also be designated, or an alternate plan of care consistent with the patient’s safety needs and hospice standards must be developed.

Hospice cares for all eligible patients regardless of their diagnosis. Eligibility is based on a prognosis of six months or less, not a specific disease. This may apply to various conditions such as cancer, dementia, stroke, renal, heart, lung, congenital defects, AIDS, or other diagnoses. Eligibility can be met through one significant terminal diagnosis that meets Medicare Disease Specific Criteria, or when multiple comorbidities contribute to a terminal decline.

ViaQuest Hospice services are designed to provide comprehensive support without financial burden. Our services are 100% covered by Medicare, Medicaid, and most private insurances, offering families peace of mind during a challenging time. This extensive coverage includes:

  • Medications: All medications related to the terminal illness and for symptom management are provided.
  • Medical Equipment: Necessary medical equipment, such as hospital beds, oxygen, and wheelchairs, is delivered and maintained at no cost.
  • Medical Supplies: A full range of medical supplies pertinent to the hospice diagnosis, including wound care dressings, incontinence supplies, and other essential items, are covered.

For patients not covered by standard insurances, the ViaQuest Foundation, our dedicated non-profit partner, may be able to assist. The Foundation is specifically established to help indigent citizens receive the comfort and care they need, ensuring essential services are accessible regardless of financial standing.

Many physicians find that the ViaQuest Hospice program greatly enhances and extends the care they provide, and our care adds quality time to their patients’ lives.

Routine Home Care (RHC)

This is the most common level of hospice care, provided in the patient’s chosen residence—your own home, a nursing facility, an assisted living facility, or a hospice house. It includes intermittent visits from our hospice team (nurses, aides, social workers, chaplains, volunteers) to provide symptom management, personal care, emotional support, and education for caregivers, delivered with gentle guidance and a focus on maximizing daily comfort and quality of life in familiar surroundings.

Continuous Home Care (CHC)

When a patient experiences a period of crisis with severe symptoms (such as uncontrolled pain or acute shortness of breath), Continuous Home Care can be provided. This involves a higher level of skilled nursing care delivered continuously in the home for a short period (typically 8-24 hours per day) until the crisis is resolved, showcasing our team’s resolute commitment and dedication to restoring comfort and stability.

Inpatient Respite Care (IRC)

Provided at a contracted nursing facility for up to five days, Inpatient Respite Care offers temporary relief for your primary caregiver—giving them the rest they need while ensuring you receive continuous expert hospice care. This level of care offers pure relief and peace of mind for families.

General Inpatient Care (GIP)

 Covered by Medicare, Medicaid, and many private insurance plans, this level involves moving you to a contracted hospital, hospice house, inpatient unit, or specialized nursing facility bed. This is used for acute symptom management that cannot be effectively achieved in your home setting, with treatment strictly focused on symptoms related to your hospice diagnosis, demonstrating our determination in ensuring optimal comfort and peaceful transitions.
